Offering Care Act Advocacy to residents of the Oldham area
Cloverleaf Oldham Advocacy Service provides advocacy to all vulnerable adults, carers and children in transition to adult services residing in Oldham and who qualify under the Care Act criteria which is set out below:
Under the Care Act 2014, a duty of care is placed on local authorities to provide independent advocates to facilitate the involvement of a person in their care assessment, for the preparation of their care and support plan and in the review of their care plan, it also includes support in safeguarding enquiries and safeguarding adult reviews, if two conditions are met:
1. The person has substantial difficulty in being fully involved in these processes. This includes
a) Understanding relevant information
b) Retaining information
c) Using or weighing information and
d) Communicating views, wishes and feelings.
2. There is no appropriate individual available to support and represent the person’s wishes.
